They care for the sick for the sake of material benefits, not out of love.

jegucchī hoti uccāraṁ vā passāvaṁ vā vantaṁ vā kheḷaṁ vā nīharituṁ;
They’re disgusted to remove feces, urine, vomit, or spit.
